I think it contains something dear to him either that or the guy who threw the balloons arms have unattached themselves from his body.

I honestly can't put my finger on it, but I assume he wants the balloon filled with water to live.

Either hes just eaten a chocolate he thought was caramel, but it turned out to be a coffee or somethings gotten a hold of his manhood.

I think the two events are completely unrelated. The man in the background looks completely calm and he is focused on the waterballoon. The man in the foreground, Mr. Weird-Face-McNegro, as he shall be called has a face of terror but his eyes are not focused on the waterballoon at all. I would venture to guess that either a panther is mauling a small child somewhere out of picture or his face is stuck like that after years of painful orgasming and an obsessive masturbation problem.
