You need episode complete for all chapters in chapter 5
I fucked one up on my second playthrough so didn't get it
It was a rgiht silly mistake I made in chapter 4 aswel where you look through those stuopid spheres, I forgot to do one somewhere

Lol. Actually...I didn't know how to complete the Calm Lands stage and just said "Fuck it" and traded the game in. XD.

As mentioned above, you'll require Episode Complete in all locations in the fifth Chapter. Obtaining 'Concluded' will disallow you access to Mascot. 
The reason you should not Complete Zanarkand last, is because in order to obtain said dressphere you need to be automatically transported to the Celcius - Zanarkand does not offer this. Any other Episode will do... just not that one.
