1) Side Quests that require perfection.
Getting gold medals on chocobo tales comes to mind. And that fukkin jump rope game on FF9.
2) Game to movie conversions
They suck 99% of the time. It's not because you can't emulate a computer game on the big screen - producers just seem to fail. Look at Silent Hill - that game has potential to be the scariest film ever in ever. I watched it with a few mates in the dark of night and nothing. We were all like 0_o wtf?
3) Movie to Game Conversions
A.K.A. Cheap tie-ins. Lets look at some examples - transformer game - SPIDERMAN 3 (OMFG) that is the biggy. The game sucked so many balls it would please all of 4chan.
4) Pointless limited editions
I recently purchased Unreal 3 limited edition. I think the box was designed by a blind four year old with one arm, a metal tin and a saw and he just went nuts. You can't open it properly without everything falling out. You have t open one side - OR the other. It's doesn't even work to open it all our for display as everything falls out.
5) Being forced to grind for longer than an hour.
Just bland.
6) Games that are a compilation of mini games and nothing more.
This doesn't include mario and sonic at the olympics which was awesome.
7) Games that only have a story because otherwise it would be a pretty painting (ff12 in particular)
8) Characters blatantly aimed for a type of people
Vincent comes to mind as he is aimed at goths/emos etc He will appeal to their nature they will become massive fanboys of him. In fact you can pretty much all the FF7 characters fall into this.
