Final Fantasy XI is similar to World of Warcraft and EverQuest in the sole reason that they're online-based games inwhich you play with other people to get more done. Otherwise, they are different in the aspect that the battles are fought differently, quests are handled differently, there are specific missions and quests to advance you through the game, and a lot more.
To answer your question specifically, the fighting is active; not turn based. Monsters will appear around you just like any other game. They will be aggressive to nothing, sound, sight, low hit points, and/or casting of spells. When you're engaged with a monster, it will continuously hit you just like any other game. The biggest difference is how you fight. It's similar to other Final Fantasy games as you can just push attack and it does the rest for you. However, it's the same idea. You can set macros for spells, special attacks, etc.
I've played nearly every MMORPG that's been released, and Final Fantasy XI is by far my favorite. A downside is that it's a bit slower than other games, but it makes it a lot more enjoyable when you reach what you're shootin' for.
