How was XIII a failure? It sold well and reviewed well. XIII-2 isn't even out in NA yet, so that's way too early to call.

The game hasn't even been officially confirmed yet, so there's no way of telling. However, I can pretty much guarantee that DLC and things of that nature from the JP version won't carry over to the NA/EU versions. Very few (if any) games allow that.

At E3 Final Fantasy Versus 13 will be announced for an autumn 2012 release in North America and announced to be renamed Final Fantasy XV. This was rumoured before, and honestly they'd be stupid not to just call it XV and be done with it.
