Could deviant practices be defined as insane just because they defy social convention so utterly? Or is insanity only and exclusively a chemical imbalance/lack of control over oneself?

I've just been thinking about the nature of madness and insanity lately since the very definition of what insane is, means different things to so many different people. I mean before we can realise if we are "insane" or "mad" we need to define it and know what to look for.
