If anyone remembers, Naomi calls Dr Clark as 'he' after the fight against Ninja/Gray Fox in MGS1. Perhaps it's something they overlooked? Or maybe Naomi never actually saw Dr Clark before. Or maybe as a way of disguising herself as a Patriot, she said her gender was Male?

Kojima has never intended to make a remake after each game. He made MG and figured it was the end...was asked to make another, same with MGS1, MGS2, and MGS3. He finally decided to make MGS4 because a lot of people were confused and he was also getting death threats.
If you go back and look at the old games you'll see that MGS4 opens a lot of plotholes for the other games. For example
Signit and the DARPA chief. Their the same person, and yet their age timeline is waaay off. They give the DARPA chiefs age in MGS1 and Signit's in MGS3 and you'll notice they don't match...there are tons of small things like this, purely linked to the fact that Kojima makes up each game as he goes, without planning ahead for another.
